Home Care Packages are designed for those with more complex care needs that exceed the capabilities of the Commonwealth Home Support Programme.
Physiotherapy funding from home care packages (HCPs) will help more seniors to remain mobile, safe, and happy at home. Many of the adults who receive these supports stay at home and stay connected to their communities while receiving funded assistance from the Queensland Government.
Seniors over the age of 65 may be eligible for funded support under the Home Care package scheme if they are assessed as able to receive clinical care in their own homes. A package of home care can range from Level 1 to Level 4, with increased monetary funding with each level.

The transition care program helps older people return to their daily activities after a hospital stay. In this case, short-term care is provided for up to 12 weeks, either in an aged care facility (nursing home), at a person’s home, in the community, or a combination of all of these.
Programs include social work, nursing support, personal care, and allied health services. State and territory governments are the approved providers of transition care.
